PATIENT CRITERIA
-
Homebound does NOT mean "unable to leave home", but only that leaving home is "taxing effort" or "must be accompanied to leave for any reason".
-
Face-to-Face encounter with MD or APP no more than 90 days prior or within 30 days after, start of care.

Need for intermittent skilled nursing, physical or speech therapy, or continued occupational therapy.
